We use to have something like this. We could perhaps put it back optionally.
This is coming soon. To answer your questions in the meantime:

1. Currently the only difference between roles is that Account Managers and Project Managers have access to the "Project Management" options. All other roles can report bugs, edit them, etc. This will change at some point in the future.

3. There is only one Account Manager. The Account Manager is the "Administrator" of the account. Project Managers have similar permissions, the only thing they can't do is add users to the Organization.
This will have to be part of a larger update that we will make once we've really mastered the "Bugs" side of things. :-)
